Applications lodged by insurance companies or solicitors

An insurance company or solicitor is able to submit a PD268 PIR or VCR application form on behalf of the applicant. This can be done without a copy of the applicant's 100 points of ID as long as the applicant signs the consent section and the paragraph below is on the cover letter:

'The identity of the person who has signed the attached PD268 has been verified by the provision of personal information or photographic evidence and this application is made with the understanding that any breach of applicable Privacy Principles may be punishable by law.'

The onus is on the insurance company or solicitor to correctly check the identity of the applicant. If an application is submitted without first establishing the correct identity of the applicant it could be considered to be providing false and misleading information and may be punishable by law. If the above paragraph is not on the cover letter then 100 points of ID with original documentation needs to be attached or the application form will be returned.
